Filling out these forms does NOT constitute enrollment, or acceptance to the facility. Space is limited, and parents cannot fill out the forms without having made previous arrangements to have child enrolled and accepted at Providence CCPS.

The forms must be completed and signed before your child may attend. You may bring the forms to your appointment to the facility, but acceptance is not guaranteed.
